Supporting Documentation – Individuals and Directors In support of your application to open an account with Shaw Markets we require the following documents. Please note these documents must be in the name of the applicant:

1. A photocopy of one form of a non-expired, government issued photo ID.
2. Your identification must clearly indicate your date of birth. All documents that contain an expiration date must be valid, unless stated otherwise.

3. A proof of residence dated within six months for the address indicated on the application. (P O Boxes are not acceptable)
   Please make sure that the billing name and residential address is visible, as well as the company that has issued the statement.
